Adult Services

As youth with intellectual and developmental disabilities transition into adulthood, Advocates continues care to guide and empower individuals to live independently, develop meaningful relationships, contribute actively to their community, with support throughout their journey. Advocates for Healthy Transitional Living employs a team of Direct Care Workers to serve Adults eligible for Long Term Care Services.  We are approved for the following provider types through the Forward Health Portal. 

 

Waiver Non-Residential Day & Vocational Services

Daily living skills training

Daily living skills training helps members do everyday tasks. This includes skills that help the member be independent and take part in community life. Examples include teaching money management, home care maintenance, food preparation, mobility training, self-care skills, and the skills necessary for accessing and using community resources.

Day Habilitation services {community based}

Day services are regularly scheduled activities provided outside the home to a group of adults. Day services help members participate in the community, learn social skills, and develop the skills needed for activities of daily living and community living.

Consultative clinical and therapeutic services for caregivers

Consultative clinical and therapeutic services help unpaid caregivers and paid support staff carry out a treatment or support plan. Services include assessments, development of home treatment plans, support plans, intervention plans, and training and assistance to carry out the plans. Services also include training for caregivers and staff who serve members with complex needs (beyond routine care).

Transportation services

Community transportation services help members access community services, activities, and resources included in their care plan. 

Non-emergency medical transportation services help members get non-emergency, Medicaid-covered medical services. Services may include reimbursement for mileage, as well as transportation of members and their attendants to destinations. 

Both exclude emergency (ambulance) transportation.

Activities

AHTL provides opportunities for our clients to have daily interaction inside and outside the setting with people from the broader community who do not receive HCBS services and are not paid to provide support.  

Each month members have the opportunity to choose from activities in integrated community settings that are published in an Activities Calendar. As these activities are planned, tasks and activities that are comparable to people of similar ages who do not receive services.  We strive to promote new learning related to areas of interest identifiedduring your person-centered planning. It is your right to decline participation in an activity if you have no interest and stay behind at the Day Program if desired and staffing is available. 

At AHTL we review client progress monthly, and more formally at a 6-month review allowing for the team to review the person-centered plan and ensure that goals and strategies are effectively implemented.  This review provides for the opportunity for schedules and preferences to be updated as you request.  

Part of developing meaningful relationships means that our your interactions are not restricted and you are able to talk and spend time with anyone you choose at the site or in the community as long as you are safe doing so.  Staff make every effort to be flexible and accommodating toyour choices of being with others. We continuously work to improve the quality of life and in line with our vision and values, we support you in making informed decisions as an autonomous, independent adult.
