Job Title: Clinical Care Coordinator

General Description:Empower children and families to change their lives for the better through a high level of determination, will and skill resulting in excellent case management 

 

Primary Responsibilities  

  • Demonstrate SHARED values
  • Support the clinical team in receiving and processing referrals and assessments
  • Timely response to referral contacts
  • Timely completion of basic information worksheet for each new referral
  • Schedule intake meetings
  • Completing intake paperwork
  • Disseminate pertinent information to Clinical Case Manager and Program Managers
  • Receive and enter client information into Kaleidacare
  • Schedule collateral meetings and appointments
  • Complete and update Behavior Support Plans (BSPs) that reflect goals for service 
  • Communicate with families and counties regarding service plans, schedules, etc. 
  • Complete bi-weekly updates on programming and goal progression 
  • Respond to requests for information
  • General administrative support, as needed
  • Direct Service with Clients

Required Skills, knowledge and abilities 

  • Ability to convey Unconditional Positive Regard to all Advocates clients
  • Fundamental knowledge of Motivational Interviewing
  • Ability to operate in a positive, dynamic, trauma-informed learning environment
  • A High level of flexibility and the ability to change and adapt quickly is essential
  • Highly organized and detail oriented
  • Ability to understand clinical needs of client
  • Excellent communication skills
  • Team oriented
  • Ability to understand client needs and issues
  • Ability to give and receive feedback
  • Ability to positively engage with individuals from diverse backgrounds
  • Ability to clearly communicate with individuals of varying communication skills
  • Ability to document and submit accurate information
  • Appropriate knowledge of mental health and social services

Experiences and Qualifications 

  • Experience with at-hope populations (preferred)
  • Pass all background checks
  • High school diploma
  • Experience with foster care systems (preferred)
  • Valid driver’s license
