Case Manager

Program: Foster Care Academy

Reports to: Program Manager

General Description: Oversee the clinical and case management services provided to foster children in their foster home, school and community.

Primary Responsibilities

  • Coordinate and/or facilitate intakes, assessment and case reviews for each foster child on caseload
  • Select appropriate foster homes for each child placed in the program, in collaboration with the Referral Coordinator
  • Review information about each child placed with foster parent, including Information for Foster Parents Parts A & B
  • Develop, in collaboration with treatment team, a comprehensive treatment plan that promotes the health and stability of the foster youth and family
  • Provide bi-weekly psychoeducation to foster families and foster youth
  • Develop and/or identify interventions and services for foster youth and family, and communicate consistently and effectively with service providers to coordinate care
  • Facilitate the development and implementation of urgent response plans for each foster child
  • Participate in on-call services
  • Ensure each child’s file strictly adheres to WI State Statutes
  • Assist in the training of foster parents, schools and communities on the needs of children in Treatment Foster Care
  • Perform necessary court responsibilities, as appropriate
  • Maintain appropriate and timely case records for each foster child in Kaleidacare

Required skills, knowledge and abilities

  1. Advanced knowledge of safety, communication and conflict resolution
  2. Advanced knowledge of children, adolescents and mental health
  3. Experience with at-risk youth and families. Preferred experience with the foster care system.
  4. Ability to coordinate and administer care for children and families in the program
  5. Ability to work early evenings (4-6 pm) and on call hours

Experience and Qualifications

  1. Pass all background checks
  2. Bachelor’s Degree in Social Work or related field and at least 2 years experience, or Master’s Degree in Social Work or related field
  3. Licensed by the State of Wisconsin (LCSW or LPC preferred)
